Formed in 2007, Elderwood Transportation provides non-emergency general and medical wheelchair transportation and is licensed to operate in the six counties of Western New York. Part of Post Acute Partners, founders Dr. Jeffrey Rubin and Warren Cole believe the most effective way to provide post acute care is to integrate health services through common ownership, offering a full continuum of care. Post Acute Partners acquires geographically concentrated companies, including Elderwood, Elderwood Health Plan and Woodmark Pharmacy in addition to Elderwood Transportation and other subsidiaries, which are already highly regarded for quality. Then they invest heavily in those organizations in the areas of clinical excellence, customer satisfaction, staff training and technology.
Elderwood Transportation leverages Elderwood’s long established expertise in long-term and subacute care throughout Western New York with its 17 facilities, 2,800 beds and 4,000 employees. Elderwood is well known to, and highly regarded by, the hospitals and physician groups in the communities that each location serves.
